Father of two-year-old girl murdered by her mother and new boyfriend says she was 'failed' by authorities who missed chances to save her

Isabelle Welsh suffered 21 broken bones in the weeks leading up to her death at her mother's home in Thornaby, Teesside, before dying shortly after a final 'massive head injury' was inflicted upon her.
